APET Foster Application and Procedures

Thank you for your interest in fostering for Animal Preservation of East Texas. Foster homes help our animals become more socialized and therefore more adoptable. The health and welfare of the animals in our care is of the utmost importance to us. Please answer the following questions to help us understand your background as well as the type of foster home you can provide to our animals. Upon receipt of the application, APET will contact you to schedule an interview and orientation to discuss the program in greater detail and answer any questions you may have.

I understand that fostering can last for a period of several weeks to several months.

I give permission for APET to contact my veterinarian to confirm vaccination and patient history.

By typing my name below, I certify that the information I have given is true and that I recognize that any misrepresentations of facts may result in my losing the privilege of fostering a pet. I understand that APET has the right to deny my request to foster an animal and I authorize investigation of all statements in this application. I further agree to the following: * To give my fostered animal proper and humane care, food, water, shelter, exercise and all other necessities. * To notify APET immediately if the fostered animal becomes ill and requires veterinary care. * If I am unable to care for my foster animal for any reason, I will contact APET immediately (903-638-6902). * To allow APET to remove any foster animal, without any notice to me, from my home or premises if I do not comply with these terms. In consideration for fostering an animal from APET, I hereby release, waive, discharge and covenant not to seek legal action against APET, its officers, servants, agents, and employees from any and all liability, claims, demands, action and causes of action whatsoever arising out of or relating to any loss, damage or injury, that may be sustained by me, or to any property belonging to me.

HOLD HARMLESS AGREEMENT
By signing this agreement, I acknowledge the potential for risks and hazards connected with fostering an animal. I am fully aware that there may be risks and hazards unknown to me connected to fostering. I hereby elect to voluntarily accept the responsibility to foster this animals(s) and engage in activities associated with caring for the animal knowing that conditions may be hazardous, or may become hazardous or dangerous to me and others. I voluntarily assume full responsibility for any risks of loss, property damage or personal injury that may be sustained by me, or any loss or damage to property owned by me, as a result of fostering an animal. I further hereby agree to indemnity and save and hold harmless APET from any loss, liability, or damages that may incur due to my participation in this activity. It is my express intent that this Release shall bind all members of my household

A. I have read the foregoing, understand it, and sign it voluntarily as my own free act and deed; B. No oral representation, statements or inducements, apart from the foregoing written agreement, have been made; C. I am at least eighteen (18) years of age and fully competent; and D. I execute this Release for full, adequate and complete consideration fully intending to be bound by same.
